Armistead Caregiver Services is proud to announce that Samantha Wendel, Armistead’s Geriatric Care Manager, has recently earned the distinguished certification of Care Manager Certified (CMC) from the National Academy of Certified Care Managers.  As a Geriatric Care Manager, Samantha is able to assist seniors and people living with disabilities in the community by coordinating services of all types.  This enables clients with chronic, functional and/or cognitive limitations to obtain the highest level of independence consistent with their capacity and their preferences for care. Some of the services that can be provided by Samantha as a Geriatric Care Manager include:

• Completing a care plan assessment, providing the client and his/her family with a detailed action plan that identifies problems and offers solutions.
• Pre-screening, arranging, and monitoring in-home help or other services.
• Conducting a home safety assessment.
• Assisting with a hospital or rehab discharge.
• Reviewing financial, legal, or medical issues and providing referrals to geriatric specialists.
• Helping with crisis intervention.
• Acting as a liaison between clients and families who do not live close by, overseeing care and quickly alerting family members to problems.
• Assisting with moving an elder to or from a retirement residence, assisted living community or nursing home.
• Providing consumer education and advocacy.

Along with assisting individuals and their families, Samantha can supplement and enhance services provided by other professionals working with seniors such as physicians, social service providers, elder care attorneys, trust officers, estate planners, probate court judges and other gerontology professionals. Samantha graduated with a BA from the University of Vermont.  She joined the team at Armistead in 2006 and has additional certifications as a Certified Senior Advisor (CSA) and PHI Trainer – Quality Jobs through Quality Care. Locally owned, Armistead Caregiver Services is a full service, private-pay, non-medical homecare company serving Vermont and New Hampshire from their offices in South Burlington, VT and Lebanon, NH.  Armistead provides personal care, meal preparation, errands, transportation, light housekeeping, medication reminders and more to seniors and people living with disabilities in a variety of settings.
