Going home after a stay in the hospital, long-term care or rehab facility can present numerous challenges. Homeward Bound offers an individualized approach to helping you get home from the hospital or rehab. We help keep you comfortable and safe in your home for the first few days and nights. Before you go home, you and your discharge team will meet with a certified Aging Life Care Professional™ to create a written Care Plan specific to your needs.
Pre-discharge we visit your home to make sure everything is ready for you. You will have fresh linens on the bed, clean towels for the bath, fresh food stocked in the refrigerator, etc. We will pick up your prescriptions and provide transportation home for you. If needed, we provide an overnight caregiver your first few nights home to provide a safety presence. We also take care of light housekeeping and meal preparation. We arrange for twice a day check-ins by a caregiver for 3-5 days post-discharge, or as needed.
Your care manager also handles coordination, communication and follow-up with your care team. We also provide transportation to any follow-up appointments. You can select from all these services on an à la carte basis. Costs vary according to services selected.
