24/7 Peace of Mind Care

If you or a loved one needs a reassuring presence in your home for safety, companionship, or for more intensive care, Armistead offers 24 hour, 7 days a week care. Daily rates are available.

Alzheimer and Dementia Care

Our caregivers can provide compassionate Alzheimer and dementia care, and respite for the family caregiver.  If your loved one suffers from memory impairment, Armistead provides weekend, vacation, daily or hourly respite care to match your needs.

End of Life Care

Armistead offers dedicated caregivers that are experienced in providing end of life care in the comfort of your own home.  We provide comprehensive caregiving services, working closely with individuals, families and community partners to provide seamless care while ensuring compassion, respect and dignity.

Nurse Supervision

We have a Registered Nurse available to assist with oversight and caregiver training as appropriate should you or your loved one need this resource. Call your local office to see if this service is right for you.

Homeward Bound

Leave the Hospital, Long-term Care or Rehabilitation Facility with Care and Confidence!

Homeward Bound is an individualized plan of care focused on keeping you safe and comfortable in your home, including:

  • Discharge planning – 2-4 hours in the hospital and your home with a Certified Geriatric Care Manager;
  • Home visit, pre-discharge, to make sure everything is ready for you;
  • Transportation;
  • Prescription pick-up and medication management;
  • Meal preparation;
  • Clean linens and towels;
  • Safety presence with an awake overnight caregiver to provide help with ambulation on your first night home;
  • Check-ins by a caregiver, twice a day for 3-5 days post discharge;
  • Communication, follow-up, and coordination with your care team;

Services can be selected à la carte.

Costs will vary on services selected.  Approximate charges are almost half the cost of a day in the hospital.  Range is $1,015 to $1,280 for the total package.

Geriatric Care Management

Making Connections-Connecting to Solutions

Armistead’s Certified Geriatric Care Managers can help you oversee care.  We offer guidance, support, problem solving, referrals, and hospital discharge assistance.  We help to arrange comprehensive support and planning through all stages of care based on individual needs.

An Armistead Professional Geriatric Care Manager (PGCM) can help by:

 Completing a care plan assessment, providing the client and his or her family with a detailed action plan that identifies problems and offers solutions.

  • Pre-screening, arranging, and monitoring in-home help or other services.
  • Conducting a home safety assessment.
  • Assisting with a hospital or rehab discharge.
  • Reviewing financial, legal, or medical issues and providing referrals to geriatric specialists.
  • Helping with crisis intervention.
  • Acting as a liaison between clients and families who do not live close by, overseeing care and quickly alerting families to problems.
  • Assisting with moving an elder to or from a retirement residence, assisted living community, or nursing home.
  • Providing consumer education and advocacy.


Right-Sizing / Down-Sizing

Transitions is designed specifically to help seniors and their families “right-size” in their current home or “down-size” for a move to a new home.

Senior Move Advisor Jana Granzella has joined the Armistead team to head the launch of this new service.

Transitions services are client-focused.  A great deal can be accomplished in very little time, saving both time and money, including:

  • Assessing the current situation and helping to develop a vision and a plan;
  • Evaluating what to keep and designing a plan for family treasures;
  • Helping to decide what will be donated, sold & discarded and making those arrangements;
  • Communicating with family members;
  • Reorganizing existing space to enhance the quality of life;
  • Exploring home maintenance concerns;
  • Eliminating safety concerns;
  • Arranging and coordinating packing and moving of furniture;
  • Unpacking and setting up your new home;
  • Related services, such as preparing your home to be sold.

Estate Dispersal:  Transitions also provides services for the dispersal of household belongings after the death of a loved one.  A death in the family introduces a whole new set of responsibilities, from removing personal belongings to the complete shutdown of an established home site.  Daily responsibilities or distances can make these tasks seem even more overwhelming.  Transitions can help make these tasks feasible and timely.  Working under direction of family members, attorneys or trust officers, the Transitions team will organize and distribute the estate accordingly.  From simple sorting to arrangements for cleaning, repair work and real estate services, this can take the pressure off already trying times and is as easy as a phone call.

Pre-Planning:  In addition, Transitions offers individuals and families pre-planning, providing direction for future dispersal of personal and household belongings.

Transitions is designed to fill in on the things that seniors and their family members can’t or don’t want to do.  It can mean a consulting session for those who can do some of the things themselves or doing each step of the move from start to finish.  Each client is different with special needs.  Transitions can do as little or as much as required.

Fees:  Charges are based on hourly rates with a sliding price scale based on level of service.  Frequently items left over in the home that are sold cover the costs of Transitions services.
